_____ Most Recent Report Card or Transfer Paper
_____ Social Security Card/SS#
*Public schools (within Florida) – must provide within 30 days.
Does not apply to Private schools – must provide upon entry.
C. FROM OUT-OF-STATE (Florida first time)

5DPT, 4OPV, 2MMR/Measles, 3 Hepatitis B,
Varicella Vaccine or Proof of disease(chicken pox)
